DA-FormMaker is a form generator for Windows from DA-Software, the label of the developers Andy Dunkel and Daniel Iwer. Website owners use it to assemble online forms from ready-made fields and from templates for contact, appointment, booking and reservation forms. The submitted data arrives as an HTML or text e-mail at one or several addresses.
Produces a working e-mail form for a website in a few minutes without coding.
Fields can be placed in any order, also side by side in several columns, and include calendar, time, file upload, popup selection and signature fields with input validation. Forms are plain HTML or responsive layouts based on Bootstrap or W3.CSS, and a captcha, an IP lock and a honeypot keep spam bots out. The finished form is exported or uploaded by FTP and works with the vendor's hosted formmail script or a copy on the site's own webspace. The trial is limited to five form fields; the Standard and Professional editions are paid.
